Primary Responsibilities:
Work with a multidisciplinary Care Coordination team
Act as the Forensic (Utah State Hospital) USH liaison for Salt Lake and Tooele counties
Coordinate the forensic USH census, including admissions, civil commitments, monthly progress, and discharge planning, where needed
Meet with forensic patients, as needed
Prepare and send monthly reports with USH information
Attend Optum USH committee meetings bi-weekly
Work in tandem with Optum civil USH liaison
Understand the civil USH process
Assist in coordinating civil state hospital discharges
Assist in coordinating referrals for civil admission
Understand and assist with Youth USH admission/discharges, when needed
Care Coordination could also include and not limited to inpatient discharges, integrated care support and member requests
Utilization management support

Required Qualifications:
Licensed Master's degree level clinician in: Psychology, Social Work, Counseling or Marriage & Family Counseling; OR Licensed Ph.D. or Licensed PsyD; OR an RN with 2+ years of experience in behavioral health (licensure must be current and unrestricted in your state of residence)
3+ years of behavioral health clinical experience in an inpatient/acute setting or outpatient setting
Driver's License and access to reliable transportation
Preferred Qualifications:
Experience in discharge planning and/or chart review
Experience in inpatient/acute setting or outpatient setting
Utilization review experience in an insurance company or managed care setting
Knowledge of Level of Care Guidelines (i.e., Milliman, InterQual, etc.)

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). No matter where or when you begin a career with us, you'll find a far-reaching choice of benefits and incentives. The salary for this role will range from $71,200 to $127,200 annually based on full-time employment. We comply with all minimum wage laws as applicable.
